CCW Failure Statistics

Where can I find data on what most commonly causes armed citizens to succumb to an attacker?For example, did they lose control of their weapon? Did they fail to disengage a safety/ rack a round?Was it poor marksmanship? Were they attacked from the side and unable to draw? Did their weapon malfunction? Were they simply unable to draw in time?
