CCW gone wrong (sadly at a child's birthday party).

This seems like the kind of problem a veteran CC would have as newer CC tend to be more uncomfortable/paranoid about such things. As much as I am a big fan of educating children in firearm safety (my own kids have many hours at the range) there is a certain age range where you can't conceptually teach firearm safety.
