Can pepper spray back fire on you? What if its not working and just pisses off the other person?

Last thing I'd want is for it to work against me. Sometimes I'll see it not work on people and that kind of scares me. Also, you could spray yourself by accident or get hurt because you need to be close to the attacker to use it. Just want some clarity on this.
