Say you are in a confined space like a train or bus and have no other option but to fight the attacker. Another component is the attacker is someone who appears to be drug addicted/homeless, thus, a good chance they have some transmittable disease. I can see it possible that strike to the jaw causing a cut or the attacker attempting to bite/scratch.
